铸造工黏结剂暴露致过敏性肺炎1例

A case of hypersensitivity pneumonitis caused by exposure to foundry binder

  • 摘要: 铸造过程中存在多种职业性有害因素可能损害劳动者的健康,其中铸造工过敏性肺炎的病例少见报道。报道1例因使用铸造用黏结剂导致过敏性肺炎的病例,该患者在某铸造企业从事模具造型工作,因“双眼刺痛、胸闷10 d余,喘憋2 d”入院,其胸部CT表现为两肺弥漫性分布的小点片状略高密度灶,肺功能显示限制性通气功能障碍、弥散功能下降;动脉血气分析提示伴有低氧血症(动脉血氧分压55 mmHg)。经脱离工作环境,糖皮质激素及西替利嗪抗过敏治疗后,患者临床症状及胸部CT小点片影消失,肺功能及动脉血气分析指标恢复正常。
